Athletic Secondary Accident Insurance
Mission CISD has purchased a limited benefit secondary accident insurance policy that covers the student - athletes while participating in UIL Athletic activities. This limited benefit secondary accident insurance is a "secondary" coverage. This plan is designed to help pay those expenses not covered / paid or payable by any other insurance. Therefore, you will be required to file first with your personal insurance covering your Mission CISD Student-Athlete. The Mission CISD limited benefit secondary accident insurance is a limited plan and will not pay 100% of the incurred medical bills or cover illnesses. Your personal insurance is the primary carrier (including Medicaid), and the Mission CISD's limited athletic insurance is the secondary carrier.
Any incurred medical bills not paid by the parent / guardian's personal insurance carrier or the secondary school insurance will be the responsibility of the parent / guardian & the student-athlete. MISSION CISD OR ITS EMPLOYEES ARE NOT RESPONSIBLE FOR THE PAYMENT OF ANY SAID MEDICAL BILLS.
Parent / Guardians must notify the coaches & athletic trainer prior to any non emergency athletic injury physician visit or the Mission CISD athletic insurance benefits may be forfeited.
A completed copy of the athletic insurance form and all physicians' notes / instructions need to be turned into the K. White Athletic Trainer after each physician's office visit please.

Mission CISD Student Athletic Accident Benefits for 2024 - 2025 is underwritten by Student Assurance Services, Inc from Stillwater, Minnesota SAS , & provided by The Brokerage Store, Inc. out of San Antonio, Texas. Student Assurance Services, Inc. has contracted for fee discounts for services received from physicians and facilities participating in the Lonestar network which is part of the USA Managed Care Organization Network (USAMCO). Please note that benefits are payable as described whether you use a Lonestar preferred provider or not. The Lonestar Network providers have agreed to accept Benefit(s) as payment in FULL when no other insurance is available. A search for these providers may be found at the Lonestar Athletic Network
The Brokerage Store also offers the parent / guardian additional insurance plans to help offset the cost of medical care for an additional fee. Additional plans include school - time accident coverage; a full -time accident coverage; dental; varsity football; primary coverage; & coverage for all UIL activities / interscholastic sports.
